APD Licensed Med Waiver Provider

 
  • All staff are background screened, trained and certified to AHCA guidelines
  • All staff trained relationship based de-escalation with no restraint
  • Residential facilities are licensed by the Agency for Persons with Disabilities, regional inspection monthly, state review annually
  • Res Hab and Companion Services are paid for by Med Waiver with no additional cost to the family
  • Room and board fees are covered by APD directly for individuals under 18 and by SSI for individuals over 18

Life Skills Development Training

  • Monday – Friday, year-round
  • Daily schedule based on the individual funding, generally 6 hours a day between 8am and 4pm
  • Transportation included at no cost to the family

Residential Care (Residential Habilitation Standard Moderate – Ext.1)

  • 24 hour care 365 days per year
  • High standards of cleanliness and supply in the home environment
  • Daily individualized goal monitoring
  • Community Engagement activities
  • Structured gentle support for life skill development in the home and community
  • Meals and snacks provided as planned by a licensed dietician (including support for special dietary requirements)
  • Full documentation and support for all medical and dental care, medication management, bloodwork and urgent care
  • Exisiting relationships with medical, psychiatric and dental professionals equipped to meet specific needs
  • Medication administered by trained and validated staff following the current prescriptions
  • Support for all aspects of personal hygiene, self care and grooming
  • Scheduling of and transportation to all medical visits and residential community outings

Our Location

Our Story

My Grandparents had an orchard of about 40 maple trees. Every spring Grandma would bundle us up and Grandpa would spend the day with us, going tree to tree, collecting the sap. We hung a bucket from each tree to catch the sap. It takes a lot of sap to make a little sugar.

Sugar Valley opened our first APD licensed group home in March 2018 in Fruit Cove, FL. We had full occupancy by December and all six clients are still with us today. In 2022 we opened our second location with five beds just up the road in Mandarin.

Today we are pleased to offer Life Skills Development Monday through Friday. Transportation is included. Each day we work on social skills, personal care, fitness and wellbeing at a variety of locations in the community.. We frequently enjoy visits to the horse ranch, pool, beach, local partner businesses, every park in the area, and wherever else the clients choose to visit. If you are on Med Waiver and have a Waiver Support Coordinator, ask them to contact us at teamsugarvalley@gmail.com We will coordinate to review the support plan and identify if we will be able to meet your needs.

If you are not yet on the Med Waiver program you will want to begin by registering with APD to start that process ASAP. Support is available but it does take time to complete the process. We are a “Standard” level program and we are not licensed for IB level care. We do not implement restraint of any kind. We do participate in MANDT training for relationship based de-escalation and we are well equipped to deal with some challenging behaviors.

For each client we complete a facility tour with the individual present. If that goes well we schedule a minimum three day trial period. At the end of the trial we discuss with the family and Support Coordinator to ensure that we are able to safely provide the level of support needed.

We are licensed to provide Respite Care and Personal Supports. At this time all of our overnight beds are occupied but we can coordinate daytime care and support through our companion program between the hours of 8am-4pm.

For Companion Services we offer a 1:3 staff ratio. For Residential Care it varies by individual from a 1:3 to 1:4 depending on the support level as determined by APD

Do you offer Adult Day Training? (Life Skills 3-4)

We are in the process of looking for a facility that will allow us to offer ADT services. We hope to find a space and open in Q1 2025
